Waiver of Liability, Assumption of Risk, and Indemnity Agreement

I, the undersigned participant (or parent/guardian if participant is under 18), acknowledge that martial arts, fitness training, and related activities involve inherent risks, including but not limited to serious injury, permanent disability, or even death. By signing this agreement, I voluntarily assume all risks, known or unknown, associated with my participation in any program at True Warrior Fitness, located at 631 N Main St, Mooresville, North Carolina, 28115.

I hereby release, discharge, and hold harmless Toby John Grear, his employees, agents, contractors, and any affiliated instructors, as well as their agents, and affiliates, from any and all claims, liabilities, or expenses (including legal fees) arising from injury, serious injury, permanent disability, death, or property damage incurred during participation, whether caused by negligence or otherwise, to the fullest extent permitted by law. This waiver applies to all future participation, training sessions, and events.

I affirm that I am physically fit to participate and will follow all safety guidelines. I understand that failure to do so could increase the risks involved, and I will not hold Toby John Grear doing business as True Warrior Fitness, or their affiliates liable for any injuries or accidents resulting from my participation.
